Position Summary

The Department of Medicine at Columbia University Irving Medical Center is seeking a dedicated and experienced Nurse Practitioner (NP) specializing in cardiovascular patient care to join our ColumbiaDoctors multi-specialty practice.

The Nurse Practitioner will practice under the clinical direction of the CDHV/CDMG Medical Director and the administrative oversight of the Chief Nurse Practitioner within the Division of Cardiology.

The Nurse Practitioner will work collaboratively with physicians, APP peers, registered nurses, other members of the care team, and administrative support staff to deliver safe, evidenced-based, patient-centered care.   

This position is located in Orange/Rockland County, NY.


Responsibilities

  • Perform and document age-appropriate comprehensive medical, surgical, and psychosocial health histories and physical examinations, noting pertinent positive and negative findings.
  • Maintain and update problem and medication lists.
  • Diagnose and manage acute, episodic, and chronic cardiovascular conditions in adult patients.
  • Order and interpret diagnostic tests, prescribe pharmacologic and non-pharmacologic treatments, and administer point-of-care testing, vaccinations, and medication administrations as part of cardiovascular services.
  • Complete recommended health screenings and collect health data in accordance with clinical protocols.
  • Perform in-office cardiac testing to include stress testing, device interrogation, bubble study, and echo with contrast studies.
  • Manage remote cardiac device clinic and anticoagulation clinic.  
  • Perform pre-op cardiac clearance evaluations and provide post-op follow-up care.
  • Coordinate, monitor, and deliver care to patients both autonomously and in collaboration with physicians and other healthcare professionals as per licensure.
  • Obtain and review internal and external records and reports to support patient care.
  • Formulate a goal-directed plan that includes appropriate medical and nursing interventions according to established protocols and application of evidence-based care.
  • Collaborate with physicians and specialists to adjust treatment plans as necessary based on patient response and clinical findings.
  • Counsel and educate patients and caregivers on health maintenance, anticipatory guidance, disease prevention, lifestyle modifications, chronic and acute health conditions, and comprehensive plan of care.
  • Perform and manage patient transitions of care, including conducting cardiovascular discharge assessments for patients transitioning from acute and long-term care facilities.
  • Assist with triaging daily patient communications, result notifications, and follow-up, including coordination with referring providers.
  • Work with multidisciplinary teams including behavioral health, social work, nutrition, pharmacy, home care, physical rehabilitation, hospice and other specialists as necessary to provide and coordinate care across the continuum.
  • Document all patient and caregiver interactions thoroughly and accurately in the outpatient electronic medical record (EMR).
  • Participate in clinical team meetings and contribute to the promotion of standardized workflows, teamwork, and exceptional customer service.
  • Assist with training initiatives for clinical staff, including skills development, in-services, and inventory management of medical supplies.
  • Demonstrate accountability and responsibility for nurse practitioner practice within the defined scope of care, ensuring the delivery of safe, competent, and high-quality patient care.
  • Contribute to the maintenance of a safe, private, compassionate, and patient- and family-centered care environment.
  • Respond to medical emergencies.
  • May be required to provide care at other ColumbiaDoctors practice sites, as needed.
  • Perform other responsibilities as assigned.

Minimum Qualifications

  • Master’s degree in Nursing as a Nurse Practitioner in the care of adults from an accredited program, plus one to two years of related experience.
  • Current New York State Nurse Practitioner and Registered Nurse licensure.
  • Must have NP Board Certification as an Adult-Gerontology Acute Care Nurse Practitioner (AGACNP), Adult-Gerontology Primary Care Nurse Practitioner (AGPCNP), or Family Nurse Practitioner (FNP).
  • Must be eligible for NPI registration.
  • Must be eligible for DEA certification.
  • Must be eligible for American Heart Association (AHA) ACLS and BLS.
  • Familiarity and proficiency with computer-based programs such as electronic medical records systems, MS Word, MS PowerPoint, and MS Excel.

Preferred Qualifications

  • At least three years of experience managing adult patients in a cardiovascular setting.
  • At least two years of prior clinical experience in a hospital or relevant setting.

Other Requirements

  • Participation in Medical Surveillance required:
    • Contact with patients and/or human research subjects
    • Wear N-95 respirator
    • Potential bloodborne pathogen exposure
    • Work with known infectious agents or biological toxins where prophylactic vaccination is available
  • Successful completion of applicable compliance and systems training requirements.
