The salary of the finalist selected for this role will be set based on a variety of factors, including but not limited to departmental budgets, qualifications, experience, education, licenses, specialty, and training. Position Summary

The Clinical Nurse Manager is a key leader in coordinating clinical services for transplant patients.  This role ensures seamless patient care, regulatory compliance and continuous quality improvement while collaborating with multi-disciplinary teams to optimize outcomes.  The position partners with the Transplant Program Medical and Surgical Director to develop strategies that enhance patient results and improve overall program efficiency.


Responsibilities:

Program Coordination & Patient Management

  • Oversee the evaluation process for potential transplant candidates, ensuring all assessments comply with regulatory requirements.
  • Manage patient pathways from pre-transplant evaluation through post-transplant follow-up, ensuring effective communication among providers, patients, and families.
  • Serve as a liaison between patients and healthcare teams, providing guidance and education throughout the transplant process.
  • Coordinate and participate in interdisciplinary selection meetings and case reviews.

Regulatory & Compliance Oversight

  • Collaborate with the Quality Assurance Team to ensure adherence to transplant-related regulations, including those set by UNOS (United Network for Organ Sharing) and CMS (Centers for Medicare & Medicaid Services).
  • Assist in preparing for regulatory surveys, audits, and accreditation processes.
  • Contribute to the development and implementation of protocols, policies, and best practices for transplant patient management.

 

Quality & Performance Improvement

  • Track and analyze Key Performance Indicators (KPIs) related to transplant outcomes, patient safety, and program efficiency.
  • Support quality assurance initiatives, including process improvement projects and root cause analyses.
  • Collaborate with internal stakeholders to synthesize clinical and operational data, identifying trends and opportunities for enhanced care delivery.

 

Operational & Administrative Leadership

  • Work closely with the Program Director to establish short- and long-term goals for the transplant program.
  • Ensure appropriate documentation, reporting, and regulatory data submission.
  • Support clinical research initiatives by coordinating patient participation and compliance with research protocols.
  • Contribute to budget planning, resource allocation, and strategic program growth efforts.

 

Patient Care / Clinical Responsibilities

  • Complete and document laboratory and radiology testing and patient follow-up; assisting in managing immunosuppression under the supervision of senior NP and Hepatologist.
  • Coordinate patient consults and transplant evaluation process for transplant candidates with the multidisciplinary team.
  • List and maintain patients on liver/intestinal transplant waitlist.
  • Review diagnostic testing, lab reports, and provide direct patient care under supervision. 
  • Collaborate with the multidisciplinary team members to determine and achieve specific health outcomes.
  • Assess patient conditions, identify nursing needs and facilitate office care and record-keeping. 
  • Participate in interdisciplinary rounds or conferences.
  • Facilitate patient education and support groups under supervision.
  • Ensure clinical standards for documentation and patient/family education are met.
  • Maintain continuity of care across disciplines and address patient/family satisfaction. 
  • Serve as liaison between patients, physicians, clinical and research teams.
  • Monitor patient progress, coordinate ancillary testing, and avoid duplication of services.
  • Communicate with related services regarding patient needs and pursue opportunities for care improvement. 
  • Other duties as assigned.

Minimum Qualifications

  • Bachelor’s Degree in Nursing, Healthcare Administration or a related field or equivalent in education and experience.
  • 4 years of related experience.
  • NYS RN License.
  • Basic Life Support (BLS).
  • Advance Cardiac Life Support (ACLS).

Other Requirements

  • Intermediate to advanced level MS Office skills and healthcare data management systems.
  • Strong leadership, problem-solving, and decision-making abilities.
  • Excellent communication and team collaboration skills.
  • Proficiency in Microsoft Office and healthcare data management systems.
  • Ability to analyze clinical and operational data for performance improvement.
  • 2 years of transplant coordination, clinical program management or a related healthcare setting.
