The salary of the finalist selected for this role will be set based on a variety of factors, including but not limited to departmental budgets, qualifications, experience, education, licenses, specialty, and training. Position Summary
We are seeking a dedicated and experienced Clinical Coordinator III (RN) to join our multidisciplinary team, specializing in the care of patients with chronic kidney disease (CKD) and end-stage kidney disease (ESKD). The RN will be instrumental in patient evaluation, education, and care coordination, ensuring optimal patient outcomes.
Responsibilities
- Conduct comprehensive evaluations of patients with CKD and ESKD to assessing their clinical needs and treatment options.
- Educate patients and their families about dialysis vascular access, home dialysis modalities (e.g., peritoneal dialysis, home hemodialysis), and kidney transplantation.
- Provide ongoing education and support to ensure patients are well-informed and actively engaged in their care.
- Collaborate with outpatient dialysis facilities to place patients in the most appropriate centers based on their medical needs and preferences.
- Coordinate with vascular surgery, laparoscopic surgery, and interventional radiology teams to expedite dialysis and peritoneal dialysis access placement.
- Act as a liaison with the renal transplant program, facilitating timely referrals and guiding patients through the evaluation process.
- Serve as the primary point of communication between dialysis centers, surgical teams, and transplant programs to ensure seamless patient care.
- Participate in weekly case review meetings with the Director of the Education Program to discuss patient progress and care plans.
- Contribute to the development and enhancement of patient education materials, care protocols, and standard operating procedures.
- Identify and address barriers to care, ensuring timely access to necessary interventions.
- Provide assistance in the ambulatory practice as needed, including support for kidney biopsies, infusions, and general nephrology patient care coordination.
- Perform additional related duties as assigned.
Minimum Qualifications
- Bachelor's degree or equivalent in education, training, and/or experience, plus four years of related experience
- Current NYS RN License (and maintenance)
- BLS and ACLS certification
- Excellent communication and teaching skills.
- Strong organizational and coordination abilities.
- Ability to work effectively in a multidisciplinary team environment
Preferred Qualifications
- Bachelor’s degree in Nursing (BSN) preferred.
- Proficiency in Spanish (spoken and written) is highly preferred.
- Proficiency in managing CKD and ESKD patient populations
- Familiarity with vascular access procedures, dialysis modalities, and renal transplantation processes.

- Participation in the medical surveillance program
- Contact with patients and/or human research subjects
- Potential bloodborne pathogen exposure
- Successful completion of applicable compliance and systems training requirements
