Position Summary

Reporting directly to the Administrative Dir & Chairman, the Clinical Coordinator II (Registered Nurse II) assists physicians with patient care, health education/promotion, electronic medical record (EMR) chart management, coordination of patient care, and clinical protocols as needed. 

The Clinical Coordinator II (Registered Nurse II) duties will include, but are not limited to, medication/immunization administration, point of care testing (POCT), clinical information abstraction, and patient education that can be in person, via our patient portal, or by phone. The Clinical Coordinator II (Registered Nurse II) will work closely with multiple providers and other clinical and administrative staff to deliver an excellent patient experience that contributes to positive health outcomes for patients and a work culture that represents our values.  This nursing position will serve as a key member of the patient care team. 


Responsibilities

  • Assesses and collects data related to patient health status and inputs this medical information into the EMR as instructed by the physician.
  • Works with the care team to develop the plan and care across the continuum (e.g. inpatient, outpatient, home, skilled nursing facility, schools) which includes assisting with medication management (e.g. medication renewals) inclusive of medical prior authorizations as needed.  Administer immunizations and medications.
  • Executes nursing interventions by collaborating with the care team and following standards of care established by the unit/department.
  •  Assumes responsibility and accountability for effectively managing nursing care of individual patients
  • Supports on-going clinical functions of the service to provide compassionate patient-centered care.
  • Handle telephone, My Chart and in-person triage (for symptom management) as well as patient education.
  • Assist with the management of urgent and emergent patient situations.
  • Coordinates multidisciplinary health care services and arranges diagnostic tests (i.e. lab, x-ray) as ordered by physicians and performs diagnostic testing, therapeutic, and interventional procedures as needed.
  • Review and act on test results (lab, x-ray).
  • Review and manage communications from internal and external “customers” inclusive of in basket messages, updating immunizations, updating health maintenance items.
  • Ensures infection control compliance within the practice.
  • Addresses appropriate measures to provide patient and family satisfaction.
  • May float to other clinics as needed to fulfill operational needs.
  • May establish standard clinical supply storage in collaboration with other clinical staff and audit that standards are being maintained. Establish standard protocols for medication/immunization inventory management and storage.
  • Participate in the development, review, and revision of associated policies, protocols, standards of care, and standard operating procedures for Urology.
  • Assist physicians with ambulatory procedures and treatments.
  • Responsible for procurement, storage, and upkeep of clinical supplies, equipment and medication.
  • Provides ongoing patient information to the interventionist and clinical team during procedures to deliver care to the patient including but not limited to cardiac monitoring, aldrete score, pain status. Monitor patient vitals (HR, BP, O2Sat, EKG), and scrubbing during procedures.
  • Other duties as assigned and appropriate to scope, education, and/or training.

Minimum Qualifications

  • Requires a Bachelor's degree in Nursing (BSN) or equivalent in education and experience
  • Minimum of four (4) years of related nursing experience
  • Current, unrestricted New York State RN License
  • Basic Life Support (BLS) certification required
  • Advanced Cardiovascular Life Support (ACLS) certification preferred within 6 months of hire
  • Basic computer skills and proficiency in MS Work and Outlook

Other Requirements

  • Ability to communicate effectively with multicultural and language-deficit patients.
  • Fluent in the written and verbal skills necessary to successfully perform the essential functions, duties, and responsibilities of the position.
  • Knowledge of medical terminology strongly preferred. 
  • Must be able to adapt to a growing and changing environment.
