Position Summary

The Clinical Care Coordinator – PA will be responsible for working along Dr. Zeeshan Sardar, MD, in the dynamic and innovative academic practice at NYP/Columbia University Medical Center and the Och Spine Hospital.

Rapidly growing departments offer collaborative practice opportunities to establish an ambulatory care practice and participate in the development of new clinical services in the Department of Orthopedic Surgery.

Responsibilities

• Treats patients at the Och Spine Hospital with Orthopedic, Neurological Spine, Rehabilitation Medicine and Pain Management issues in both the inpatient and outpatient settings and/or other situations.
• Work independently or collaboratively with the Attendings, Fellows, and Resident staff as a well as the nurses, therapists, social worker’s and other support personnel who take part in caring for orthopedic spine surgery patients.
• Care coordination for patients needing non-operative treatment
• Assist in managing pre-operative care across other practitioners to optimize patients for surgery
• Obtain relevant consultations from other services and follow up on the information.
• Peer-to-peer consults to obtain surgical pre-authorization for testing or surgeries for all spine surgical patients
• Rounds independently and/or with attendings on all orthopedic inpatients.
• Chart preparation according to protocol and orders appropriate x-rays for office hours using the Electronic Medical Record.
• Complies with Meaningful Use Measures.
• Evaluates and diagnoses patients
• Orders and interprets diagnostic and therapeutic tests relative to patient's age-specific needs.
• Administers injections/Prescribes appropriate pharmacological and non-pharmacological treatment modalities.
• Monitors effectiveness of prescribed intervention.
• Remove/apply dressings, sutures, staples, slings and/or braces
• See post-op patients for wound checks and post-op care and notify attending of findings if abnormal
• Reviews post-operative discharge instructions with patients including regarding wound care and pain management.
• Returns patient phone messages at designated times throughout the day; documents action taken in the Electronic Medical Record (EPIC).
• Triage incoming telephone calls; instructs patient and family regarding treatment instruction, then documents action taken in the Electronic Medical Record (EPIC).
• Oversees appointment bookings and ensures emergency situations are scheduled appropriately
• Sends patient prescription requests to pharmacy and documents action in the Electronic Medical Record (EPIC).
• Manage and oversee all spine attending in-basket tasks in EPIC
• Fill out necessary forms, i.e. disability forms, workman's comp, handicap parking as needed; Assist with patient correspondence as needed, i.e. letters of medical necessity, school notes, insurance inquiries, etc.
• Assist patients in meeting health needs by identifying potential or existing problems, exploring available alternatives and providing support to the patient and family members to make an informed decision regarding health care.
• Facilitates the patient's transition within and between healthcare settings, e.g. admitting, transferring, discharging and home care including visiting nurse referrals.
• Formulate patient care protocols and critical pathways.
• Assist with development; participate in design and implementation of clinical research trials.
• Participate in accordance with hospital policies.

Minimum Qualifications

Minimum education:

• Physician Assistants:  Graduate of an ARC-PA accredited or New York State Education department approved PA education program.

Minimum related experience:

• PA - 5 years nursing or related experience.

Minimum licensing/certifications required upon start date:

• Current NYS PA License and NCCPA Board Certification.
• BLS Certification
• DEA Certification

Preferred Qualifications

• Clinical doctorate; prior experience in management of adults with orthopedic diseases in an orthopedic spine practice, physical medicine and rehabilitation practice or pain management practice; CPR/AED certification, Experience with phlebotomy and spinal injections; Bilingual (Spanish/English).
