The salary of the finalist selected for this role will be set based on a variety of factors, including but not limited to departmental budgets, qualifications, experience, education, licenses, specialty, and training. Position Summary:

A candidate with a Master of Social Work (MSW) is sought for the Department of Neurology, General Neurology outpatient practice.


  • Provide psychosocial support and care coordination for people with neurological conditions and their families.
  • Complete forms required for referrals, including homecare, rehab therapy, transportation, disability, long-term care, FMLA, and M11Q.
  • Assist with ordering durable medical equipment.
  • Link patients to mental healthcare services as needed.
  • Connect patients to concrete services such as housing, cash assistance, insurance, and patient assistance programs.
  • Perform crisis management for patients with acute needs.
  • Identify community resources to help promote patient and family quality of life.
  • Provide psychoeducation regarding condition, treatment plan, healthcare benefits, and insurance.
  • Participate in staff meetings, in-services, and administrative activities.

Minimum Qualifications:

  • Must hold a master’s degree in social work and 3 years of related experience.
  • Certificate from the New York State Education Department.
