Position Summary
The Family Navigator (FN) position was created to work in tandem with Promise Education Coordinators, clinicians and hospital staff, serving as an intermediary between parents and their child’s service providers as well as schools. Family Navigator services are available to select Promise patients and their families to assist those families identified as needing intensive case management-like guidance to obtain needed educational services as well as other medical/intervention supports. This FN position will also cover these services under the Early Promise initiative, including providing direct service linking parent/guardian with appropriate DOE and community based aids, consulting with partner community based organizations (CBOs), being a liaison between Promise, families, and private schools for students with learning disabilities, and attending Early Promise team meetings.
