The salary of the finalist selected for this role will be set based on a variety of factors, including but not limited to departmental budgets, qualifications, experience, education, licenses, specialty, and training. The above hiring range represents the University's good faith and reasonable estimate of the range of possible compensation at the time of posting.
 

Position Summary

The Diabetes Nurse Practitioner will be responsible for the assessment and management of hospitalized adult patients with diabetes.


Responsibilities

  • Provide critical transition care for adult patients transitioning from inpatient to outpatient care as well as to other adult patients at high risk for diabetes complications such as those who have been lost to follow-up, those who have recently missed scheduled appointments and those with poorly controlled diabetes.
  • Work closely as part of the diabetes care team with team members from multiple disciplines.  
  • Responsible for the assessment and management and education of adult patients with diabetes in the inpatient setting.
  • Responsible for diagnostic and therapeutic interventions regarding insulin initiation and insulin adjustments for patients who are treated with multi daily insulin injections or insulin pumps.
  • Complete patient's history and physical, review laboratory results and providing recommendations for ordering of appropriate tests Provide diabetes education to other specialties throughout the Medical Center.
  • Perform other duties as requested.

Minimum Qualifications

  • Bachelor's degree or equivalent in education and experience, plus four years of clinical experience.
  • Current New York State NP and RN license.

Preferred Qualifications

  • Master's degree in Nursing.
  • Previous Nurse Practitioner experience a plus.
  • Adult diabetes experience.

Other Qualifications

  • NYS NP License, maintain and renew when necessary. (NP’s are required to have a NYS license to qualify for the position).
  • New York State RN License.
  • NP Board Certification.
  • DEA license.
  • BCLS and ACLS certification.
