Position Summary

Columbia University Medical Center, Department of Surgery, is seeking a Nurse Practitioner (NP) to work within the outpatient practice of the Division of Surgical Oncology under the leadership of the Division Chief.


Responsibilities

  • The NP will render direct patient care to outpatients including but not limited to: Obtaining patients' comprehensive history, performing comprehensive and targeted physical examinations, ordering and evaluating diagnostic tests, diagnosing/managing chronic illness as well as acute patient problems, and complex medication adjustment.
  • The NP will also be responsible for counseling and educating patients and their caregivers on disease processes and care management outside of the hospital.
  • The NP will be expected to collaboratively and autonomously facilitate and deliver care to this specific subset of patients.
  • The NP will obtain and review outside records/reports as needed and follow-up/collaborate with the Physician to re-develop the treatment plan as necessary.
  • The NP will work with his/her peers and support team to triage daily patient calls and needs.
  • The NP will communicate with referring Providers and other internal and external healthcare professionals and agencies.
  • The NP will also be expected to work with the multidisciplinary team including but not limited to Social Workers, Nutritionists, Pharmacists, Physician Assistants, the Palliative Care Team, inpatient Providers, and others as necessary to provide/coordinate care across the continuum.
  • The NP is required to document all patient/caregiver encounters in the outpatient EMR.
  • The NP is expected to participate in weekly multidisciplinary Team meetings.
  • Assists in development, implementation, and evaluation of standards of care and professional practice in collaboration with members of the health care team and in accordance with institutional policies and regulatory agency requirements.
  • Formulate a goal-directed plan that includes health maintenance, anticipatory guidance and appropriate medical and nursing interventions according to the established protocols.
  • Facilitate hospital admissions, transfers, and discharges as well as coordinate appropriate inpatient and outpatient accommodations and home-care.
  • Participate, review, disseminate, and incorporate research findings into clinical practice.
  • Perform patient record reviews with collaborating physician in a timely fashion.
  • Communicates effectively and professionally with patients, significant others, and members of the health care team about the patient's plan of care.
  • Demonstrates responsibility and accountability for Nurse Practitioner practice within their respective scope of care and the delivery of safe, competent patient care.
  • Participates in maintaining a safe, therapeutic, and consumer-oriented environment.

Minimum Qualifications

  • Requires a Master's degree in Nursing as a Nurse Practitioner and board certification as an Adult Nurse Practitioner (Adult Gerontology Acute Care Nurse Practitioner (AGACNP-BC) preferred) with 2 years of related experience.
  • Current New York State Registered Professional Nurse and Nurse Practitioner licenses.
  • Advanced Practice Registered Nurse is either masters or doctorate prepared as a Nurse Practitioner, as well as board certified with ANCC or AANP. The Nurse Practitioner must have a valid license as a Registered Nurse and as a Nurse Practitioner.
  • Current, or be eligible for, a DEA license for New York State
  • BLS Certified
  • ACLS Certified

Other Requirements

  • Experience with the advanced Heart Failure, Pulmonary Arterial Hypertension, and/or cardiac transplantation patient populations preferred as well as those patients being considered for/undergoing advanced heart failure therapies such as outpatient intravenous and/or inhaled medication support, and mechanical circulatory support devices.
  • Ability to manage and coordinate a patient complex caseload.
  • Work within a collaborative agreement/protocol with a physician and health care team.
  • Demonstrate leadership ability to manage patients and interact effectively with peers and physicians.
