Position Summary

The Clinical Coordinator II will function in the outpatient setting of the Kidney/Pancreas Transplant Program in collaboration with the Kidney/Pancreas Transplant multidisciplinary team to care for and manage post-transplant kidney and pancreas recipients, post-operative kidney living donors, and patients who underwent surgical or invasive procedures by the Kidney/Pancreas transplant surgeons.


  • Coordinates outpatient care of transplant recipients including the review of diagnostic testing, and lab reports as well as direct patient care under the supervision of the attending and NP/PAs.
  • Facilitate communication and coordination between all members of the healthcare team, involving the patient and family in the decision-making process, to optimize patient outcomes.
  • Educate the patient about transplant pre and post care management, assists with prior authorizations and responding to patients calls.  
  • May collect and audit data as required and provide support for scheduling of care and coordinator of care.
  • Collect and interpret lab work and diagnostic studies under agreed upon protocols.
  • Order and obtain medication prior authorizations for kidney transplant recipients.  
  • In partnership with Quality, Research and Education group, help develop and manage detail clinical databases to evaluate and improve patient outcomes.
  • Provide compassionate patient-centered care.
  • Attend professional meetings and conferences, maintain professional affiliations and enhances professional development to stay current with trends in health care administration.
  • Performs other special projects and duties as assigned.

Minimum Qualifications

  • Minimum education:

    Bachelor’s degree or equivalent in education and work experience required.

    Minimum related experience:

    4 years

    Minimum licensing requirements:

    New York State RN license required.

    Minimum certification requirements:


Minimum computer skills:

Intermediate MS Office skills

  • Must be able to successfully complete systems training requirements.
