Position Summary
The Naomi Berrie Diabetes Center at the Columbia University Irving Medical Center is seeking a competent and compassionate registered nurse. The Diabetes Clinical Coordinator I - RN provides nursing support for patients with diabetes and diabetes clinical trial participants. This role involves patient education, monitoring, and coordination of care to support disease management and improve patient outcomes. The nurse works closely with endocrinologists, diabetes educators, and other team members to deliver comprehensive, compassionate care. The Clinical Coordinator I - RN will work closely with multiple providers as well as other administrative staff to deliver an excellent patient experience that contributes to positive health outcomes for patients and a work culture that represents our values.
Responsibilities

- Perform assessments including vital signs, blood glucose checks, and medication reconciliation.
- Assist with diabetes-specific evaluations, including insulin administration, glucose log and CGM review, and insulin pump data review.
- Assess and collect data related to patient health status an input this medical information into the EMR as instructed by the physician.
- Educate patients and families on blood glucose monitoring, CGM, insulin use, dietary guidelines, and lifestyle changes.
- Monitor A1C trends and support adherence to diabetes management plans.
- Support patients in understanding medication regimens and side effects.
- Triage patient calls and provide guidance related to hypo/hyperglycemia or medication issues.
- Assist with referrals to diabetes educators, dietitians, ophthalmologists, and podiatrists as needed.
- Perform phlebotomy and IV placement as needed for patients and clinical trial participants.
- Support the conduct of diabetes clinical trials through performance of study-related assessments and specimen collection, processing, and shipping.
- Maintain accurate documentation in the Electronic Health Record (EHR).
- Ensure proper documentation for insurance, medication refills, and care plans.
- Participate in chronic disease management initiatives and quality improvement programs.
- Under supervision, execute nursing interventions by collaborating with the care team and following standards of care established by the center.
- Provide diabetes education for hospitalized patients as needed.
- Perform general administrative functions, and related duties and responsibilities as assigned/requested.
Minimum Qualifications
- Bachelor's degree in Nursing or equivalent in education and experience.
- 0-2 years related experience. Clinical rotation experience associated with BSN degree programs will be applied toward related experience requirements.
- Current New York State RN License.
- BLS certified.
Preferred Qualifications
- Experience with diabetes or chronic disease management is highly valued.
- Certified Diabetes Care and Education Specialist (CDCES).
- Proficient in EHR systems and documentation.
Other Requirements
- Strong patient education and counseling skills.
- Familiarity with diabetes technology including glucose meters, insulin pens, CGMs, insulin pumps.
- Ability to handle confidential information and maintain confidentiality according to HIPAA.
- Must have excellent communication skills and the ability to communicate effectively with a diverse audience of peers, management and other stakeholders. Must have highly developed interpersonal skills and ability to deescalate stressful situations.
- Must be an engaged and positive team member demonstrating collaboration, inclusion, reliability, punctuality, and adaptability to new and changing situations including helping peers adapt to new situations.
- Strong proficiency in computer skills including, but not limited to, Electronic Health Systems, Microsoft Excel, Microsoft Word, Email systems, etc.
- Must be a motivated individual with a positive mindset and exceptional work ethic.
