Position Summary

The Adult Diabetes Nurse Practitioner will be responsible for the assessment, management and diabetes education for Naomi Berrie Diabetes Center adult outpatients in addition to hospitalized adult patients with diabetes.


Responsibilities

    • Responsible for the assessment and management and education of adult patients with diabetes in the outpatient and inpatient setting.  Responsibilities including diagnostic and therapeutic interventions including insulin injection technique, glucose meter teaching, as well as insulin pump and sensor starts and ongoing diabetes education to patients, families and caregivers.
    • Complete patient's history and physical, review laboratory results and order appropriate tests, provide treatment recommendations, including insulin initiation and dose adjustment recommendations, discharge instructions as appropriate and patient education.
    • Provide critical transition care for adult patients transitioning from inpatient to outpatient care as well as to other adult patients at high risk for diabetes complications such as those who have been lost to follow-up, those who have recently missed scheduled appointments and those with poorly controlled diabetes.
    • Help coordinate the pediatric to adult transition program and work closely with the pediatric diabetes team to assure the seamless transition of the pediatric diabetes patients to the adult service, a time when many patients with chronic illnesses often have significant life-threatening gaps in healthcare.
    • Work closely as part of the diabetes care team in the outpatient center and the hospital and collaborate closely with team members from multiple disciplines.  
    • Provide diabetes education to other specialties throughout the Medical Center.
    • Fulfill electronic and faxed prescriptions for diabetes medications and supplies, review lab results with patients, answer phone calls and patient portal messages regarding diabetes management, complete forms and letters as requested
    • Perform other duties as requested.

Minimum Qualifications

  • Bachelor's degree or equivalent in education and experience, plus four years of clinical experience.
  • Current New York State NP and RN license.

Preferred Qualifications

  • Master's degree.  
  • Adult diabetes experience.
