Position Summary

Under the oversight of the Chief Nurse Practitioner for the Center for Advanced Cardiac Care (CACC), the Clinical Coordinator will work directly with the Heart Failure/Transplant Nurse Practitioners and Physicians. The Clinical Coordinator will implement nursing care in the outpatient setting. He/she will be based in office and clinic settings for heart failure and heart transplant patients.


  • Obtain and review laboratory and other diagnostic test results.
  • Evaluate symptomatic patients and conduct telephonic triage.
  • Provide initial and ongoing patient and caregiver education including but not limited to medication and lifestyle management and disease process.
  • Administer medications/treatments as ordered by the MD and/or NP.
  • Responsible for clinical documentation in medical records and for remote monitoring as prescribed by the MD and/or NP.
  • Perform the duties stated above in close collaboration/communication with the NPs and MDs of the CACC.
  • Perform other responsibilities as assigned.

Minimum Qualifications

  • Requires a Bachelor's degree in Nursing (BSN) or equivalent in education, plus 4 years of related clinical experience. A Master's degree can substitute in part for the required experience.
  • Graduate of an accredited Registered Nurse program from an accredited college or university.
  • Current New York Registered Nurse license, BCLS and ACLS certification with recent cardiology experience required.
  • Excellent clinical, interpersonal and communication (both written and verbal) skills required.

Preferred Qualifications

  • Spanish proficiency is desired.

Other Requirements

  • Participation in medical surveillance is required. 
  • Must successfully complete systems training requirements.
